SEASON 2012-2013 *Schedule Exemption and Travel Permit to be submitted by rostered “Bench Staff” ONLY* *Approved Travel Permits will be returned to the person named on the permit, by email within TWO WEEKS of travel date* • Teams planning to attend a tournament within the Branch (Manitoba) shall complete this form and obtain approval from Hockey Winnipeg. There is NO CHARGE for a permit for a tournament in Manitoba. • Teams planning to travel outside of the Branch (outside Manitoba) to attend a tournament or to play an exhibition game(s) require a HOCKEY MANITOBA TRAVEL PERMIT. The team must first obtain the signed WMHA Travel Permit before applying for the Hockey MB Travel Permit. THE HOCKEY MANITOBA TRAVEL PERMITS ARE ONLY AVAILABLE FROM THE HOCKEY MANITOBA OFFICE AT 145 PACIFIC AVE. YOU WILL BE REQUIRED TO PAY HOCKEY MANITOBA $20.00 FOR THE PERMIT. • Teams traveling outside of Canada are advised to arrange for additional medical insurance coverage. All players traveling to the USA MUST have some form of primary insurance to be eligible for Hockey Canada’s supplemental insurance. It will be the responsibility of the team applying for a Travel Permit to PRINT and RETAIN a copy of the permit and the return email (confirmation) stating that the exemption has been received by email, by the correct date.
